2017 Lay Pension Reporting

Every parish in the Diocese of Ohio must submit a lay pension report to the Finance Office on or before October 1, 2017.

This year we are continuing to use the Adobe fill-in forms OR you may use the Word/Excel formats.

Completed forms can be submitted electronically to the Finance Office using the PDF fill-in format or submitted to the Finance Office by mail or FAX. Our FAX number is 216-623-0735, or mail to:

Parishes must submit a lay pension report in order to retain vote for their lay delegates at Diocesan Convention. Parishes which do not submit a completed form retain seat and voice for their lay delegates, but lose voting privileges.

If you have lay employees currently participating in a lay pension plan, please compete Lay Pension form #3 as described below, and also submit a copy of your lay pension form from Church Pension Group (CPG) for the month of July. This form, or other documentation, will verify that lay pension payments are paid through July 31st of this year.

Lay Pension Form #2 - Complete this form if the parish has lay employees, but  all lay employees work less than 20 hours per week.

Lay Pension Form #3 - Complete this form is the parish has at least 1 employee who works at least 20 hours per week.
